Your First Internal Pelvic Floor Exam: What to Expect

I know—those words alone can sound intimidating. But here’s the truth: an internal pelvic floor internal exam is NEVER required and is simply a way for me to gather the most accurate information to help you feel better, faster. You are always in control of what happens and you’re welcome to change your mind at any time.

1. We Start with a Conversation
Before anything physical happens, we’ll talk about your symptoms, history, and goals. You can ask every question you need—nothing is too small.

2. You Have Options
An internal exam is never required. We can assess your pelvic floor externally or through feedback-based methods. I will offer a menu of options for you to choose from based on your comfort level.

3. What It Looks Like
An internal exam is a collaborative process, I will explain each step and check in with you often to ensure your comfort and consent throughout. If you choose to have an internal assessment, you will be comfortably positioned on your back with a sheet draped over your lower half. Often, pillows can be positioned to ensure your comfort. I will use one gloved and lubricated finger to gently assess your pelvic floor muscles internally—checking strength, coordination, and tenderness. I will assess your ability to contract and relax the pelvic floor muscles, noting any limitations or weaknesses. No speculums. No stirrups. No rush.

4. Why We Do It
This helps me pinpoint what’s going on in your actual pelvic floor muscle tissue so we can make a plan that’s truly tailored to you. I will be able to determine if muscle tension, weakness, incoordination or a combination of these is causing your symptoms. Many people say it feels far less awkward than they expected.

Your comfort is the priority. The goal is always to empower you with information—not pressure you into anything.
